This information should be of a unique or special nature such as a medical issue, physical limitations, learning needs, social-emotional situations, cousins in the same grade level, etc. YOU MAY NOT REQUEST A SPECIFIC TEACHER. This is for unique information impacting placement only.
You can submit this form for only one student. However, you can submit it multiple times if there are other students for whom you have unique information to share. This information remains confidential and is seen only by the Principal, Assistant Principal and School Counselor.
This information is not retained nor is it referenced for the following school year. You must share this unique information each year for it to be considered when classes are being built.
