If your student has medication at school: Medications cannot be sent home with your
student; therefore, please plan to pick up your student’s medication(s) from
school by the end of the school day on Thursday, May 26th.
Any medication not picked up by the end of the school day on Thursday, May 26th
will be disposed of in accordance with the state law.
Families of students with asthma, food allergies or bee allergies: In planning for next year, asthma packets, food allergy packets, bee allergy packets and the Request for Medication Administration by School Personnel form are available for the Health Services website http://www.chagrinschools.org/HealthServices1.aspx. Please complete the appropriate packet(s) and/or medication administration form and bring them to school, along with the medication, by the first day of school. We ask that all forms and medications be turned in by the first day of school to help to assure the safety of your child.
