MEDICATION ADMINISTRATION AT SCHOOL

REQUEST FOR MEDICATION ADMINISTRATION BY SCHOOL PERSONNEL must be completed and signed by both parent/guardian and prescriber.

This is for both over-the-counter (OTC, e.g. Advil, Tylenol) and prescription medications.

Completed form(s) and medication(s) must be brought to school by parent/guardian.

STUDENTS ARE NOT PERMITTED TO CARRY MEDICATIONS WITH THEM, with the exception of EPI-PENS, INHALERS and DIABETIC SUPPLIES & MEDS, as prescribed by a health care provider.

Prescription medications need to be in the original container labeled with the student's name.

OTC medications need to be in an original, unopened container.
